Progressive Responsibility EM-1

EM-1 residents provide direct care to patients under the supervision of an emergency medicine attending and EM-3 resident. We encourage the EM-1's to formulate a comprehensive differential diagnosis and plan prior to case presentation to the senior resident or attending. Advanced procedures, such as intubations and central venous lines, are directly supervised by an attending.

 

Clinical responsibilities

  1. Perform an accurate history and physical examination.
  2. Generate a thorough differential diagnosis, assessment and plan.
  3. Become proficient at all important emergency department procedures.
  4. Present patients to the attending in an accurate and concise manner.
  5. Demonstrate ability to interact with patients and their families in a professional a compassionate fashion.
  6. Function comfortably as a member of the entire ED team which includes attendings, coresidents, nurses, physician assistants, clerks and other ancillary staff.
  7. Communicate effectively and collegially with residents and attending physicians from other specialties.
  8. Demonstrate an ability to clearly and completely document on the chart in legible handwriting the history, physical exam, clinical course, assessment and plan of each patient.
  9. Formulate appropriate discharge plans and communicate them to patients and family.
  10. Develop regular follow-up of selected admitted and discharge patients.

Supervisory responsibilities

None

 

Teaching

1.       Attend 75% of emergency department weekly conferences, trauma conference, journal club and grand rounds.

2.       Develop and deliver 2 case conferences under the supervision of the associate residency director and faculty advisor.

3.       Present the pre-hospital course and ED management of trauma cases at the joint EM/Surgery Trauma conference as requested.

4.       Teach medical students from Columbia University College of Physicians and other medical schools.

5.       Obtain ACLS, ATLS and PALS certification.

6.       Prepare mini-conferences and Board Review questions and answers as assigned.

 

Administration

  1. Become familiar with the physical structure, equipment locations and chart/patient flow patterns of the emergency department. Learn functions of all personnel assigned to the emergency department.
  2. Complete death certificates and discuss cases with the New York City Medical Examiner.

Research

  1. Attend journal club and present summaries of assigned articles including methods papers. Learn research methodology, statistical analysis, and critical assessment of the medical literature.
  2. Participate in ongoing research projects while on duty in the emergency department.
  3. Formulate a plan for a scholarly activity project. (see research section of residency manual)
