Please be advised that because of the high demand for this program, not all volunteers can be accommodated. Priority will be given to returning volunteers.

Fill out the application, save it, and print it. Except for the greyed out fields, Fill in the greyed out (highlighted) fields by hand Identifiable information and signatures should be filled out by pen.  Fill everything out online.

Obtain a form from your class dean (CC and SEAS) or premed advisor (GS and Barnard) confirming that you are in good academic standing. Attach this to page 3 of the application.

Make an appointment with your physician or health care provider Ask them to fill out the vaccination form for you - page 5 of the application.

Note: If you have not had a PPD test in over a year, you will be required to get one. You can go to the Employee Health Office at St. Luke's (enter on south side of 114th and ask the guard for directions). You can only get it done on Mondays and Wednesdays because it must be read two days later (on Wednesday or Friday). Alternatively, you can have your private doctor do it.

Drop your paperwork off with Kelly Davis at 101 Carmen Hall. The deadline for the application is noon on Friday, May 12th. It is in your best interest to give in your application as soon as possible as we schedule individuals on a first come, first serve basis.

Attend the Mandatory Volunteer Orientation The Summer 2006 Orientation will be on Thursday, June 8th at 6pm. It will be held in the Muhlenberg conference room 4C. (Enter on 114st and Amsterdam).

Do not forget to attach a copy of your social security card and your driver's license (or student ID) to page 4 of the application.
